Self-administered but not self-scored, the Health Risk Assessment is based on U.S. Centers for Disease Control and Prevention algorithms.
For provider and customer convenience, the Health Risk Assessment is available in three mediums:
Paper: Traditional, mailed to homes or available at hospitals, health fairs, physicians' offices. Printed results mailed confidentially to respondent's home.
Online: Interactive version available via provider's CRM Health PortalTM or website at home, hospitals, physicians' offices or health fairs. Immediate results generated as web page.
Palm: Wireless version can be programmed onto Palm® devices for easier availability at health fairs, employer sites and physicians' offices. Results available via mail or in some cases, via employer health portal.

The Health Risk Assessment is one of two assessments that form a comprehensive assessment survey tool for employees in the workplace. The other is a Workplace Assessment to evaluate the work culture and environment, employees' balance between work and life, and employee loyalty.
As part of the Health2WorkTM Program, both the Health and Workplace Assessments measure the costs associated with healthcare, absenteeism and turnover to assist employers in managing these issues. This information, provided in aggregate reports, can help employers to establish baseline costs, compare those costs to norms and then to monitor improvement.
